If you’re quoting underwritten products, you will typically need Employees to complete the Medical Health Questionnaire (MHQ). However, there may be times when you don’t need this information. If this is the case, you can disable the MHQ to remove it from the Employee Interview.

How to disable the medical history questions
- Go to the Proposal Listing.
- Locate your client’s Proposal and select the 'Edit' button.
- Navigate to Step 3: Proposal Configuration.
- Locate the Workflow Options section and uncheck the 'Require Medical History' option.
- Save and continue.
When the Employees log into their accounts, they will see only two items under the Health Questions section. Employees are still required to answer questions regarding Tobacco and Disabilities.
How to re-enable the medical history questions
- Go to the Proposal Listing and locate your client’s Proposal.
- Select the ‘Edit’ button to access the Proposal.
- Navigate to Step 3: Proposal Configuration.
- Locate the Workflow Options section.
- Re-check the 'Require Medical History' option.
- Save and continue.
NOTE: If Employees have already signed their accounts prior to the 'Require Employee Medical History' box being checked, they will be required to re-sign their accounts.
What happens to an Employee's previously entered medical information?
The medical information will remain in the user's account, but won't be displayed or mapped to any Carrier form. The information could be used for quoting self-funded insurance.
NOTE: When this is turned off, Employees cannot update any medical information that was previously added to their account or add any new medical information.
How does FormFire designate forms that exclude medical information?
Forms that map without medical conditions are indicated with a tilde (~) in the form name (e.g., ~CarrierName 2-50 OH).
NOTE: Selecting a form without a (~) will result in medical conditions mapping to that form.
