This article walks through how to configure a Proposal when quoting medically underwritten products for an Employer. It covers setting up the Medical Health Questionnaire (MHQ) in the Employee Portal, choosing appropriate Coverage Lines, configuring Workflow Options for Prescreen, Plan Selection, and QLE, and preparing Employer and Employee communications. These steps take place in Step 3 of the Proposal, also known as the Proposal Configuration step.

Workflow Dates
- Effective Date: The date when benefits and coverage begin.
- Renewal Date: Used in some Carrier Forms and the Upcoming Renewals report.
Coverage Lines
Coverage Lines represent the insurance benefits available in the Proposal. Select Coverage Lines based on the Employer’s needs. These selections carry over to the Employee Coverage Lines step in a Proposal.
Note: Enabling Medical, Life, Short-Term Disability, or Long-Term Disability Coverage Lines also enables their corresponding Prescreen Workflow Options.
Require MHQ
The MHQ is a set of questions in the Employee Portal that collects Employee health information for Carrier underwriting. To require the MHQ, check the ‘Require MHQ for Employees’ box in the Proposal. This unlocks additional MHQ settings.
Prescreen
Prescreen dates define the window during which Employees can access the MHQ in the Employee Portal. Setting open and close dates ensures:
- Employees complete the MHQ on time
- Submitted information can’t be changed while you’re quoting
When choosing dates, allow enough time between Prescreen, Plan Selection, and Qualifying Life Event (QLE) periods.
- Medical Health Questionnaire Open Date: Employees can begin filling out the MHQ survey.
- Medical Health Questionnaire Close Date: Employees can no longer make changes to the MHQ survey.
Prescreen Workflow Options
These Workflow Options determine what appears and is required in the MHQ section of the Employee Portal.

Require Employee Medical History: Activates collection of Employee medical conditions.
- Typically used for Prescreen and Plan Selection when quoting underwritten products
- Can also be used for QLE to speed up next year’s enrollment
- Required if the Medical Coverage Line is enabled

Require Employee Medical History When Waiving: Requires Employees to complete medical history even if waiving coverage for themselves or dependents.
- Helpful for Prescreen, Plan Selection, and QLE
- Required if the Medical Coverage Line is enabled
- Must also enable Require Employee Medical History

Require Employee Total Household Income: Collects total household income, which helps determine eligibility and premiums for ACA plans.
- Rarely used—typically only for Prescreen if this information is needed

Require Employee Life with Beneficiaries: Auto-enrolls Employees in Life coverage and requires beneficiary information.
- Typically used for Plan Selection when offering 100% Employer-paid Life insurance
- Required if the Life Coverage Line is enabled

Require Employee STD: Auto-enrolls Employees in Short-Term Disability coverage and requires beneficiary information.
- Typically used for Plan Selection with 100% Employer-paid STD
- Required if the Short-Term Disability Coverage Line is enabled

Require Employee LTD: Auto-enrolls Employees in Long-Term Disability coverage and requires beneficiary information.
- Normally used for Plan Selection if the Employer is offering 100% Employer-paid LTD coverage.
Needed if the ‘Long-Term Disability’ Coverage Line Option is enabled.
- Normally used for Plan Selection if the Employer is offering 100% Employer-paid LTD coverage.
If you're doing Enrollment in Quote+, you'll also need to configure Enrollment Workflow Options in the Enrollment Hub after closing the Proposal. Learn more about configuring Enrollment Workflow Options.
Employer information and contact information
These sections are pre-filled with data from the Proposal’s Employer Basics step. Take a moment to review and confirm that the information is accurate before continuing.
Employer communication
Employees need an account to log in to the Employee Portal and complete the MHQ. To create an account, they must use an Employer Code, which is automatically generated and included in the Employer Communications.
- The HR section lists Employees with the HR Manager role. These Employees receive communications with role-specific information.
- The Employees section lists all other Employees. Be sure to select the HR Managers and Employees who should receive communications.
If no Employees were added during the Proposal’s Employee Demographic step, you can enter the name and email of the Employer’s primary contact or HR representative in the Additional Individuals section. That person will be responsible for forwarding the communication to the rest of the company.
To preview a letter with the Employer’s information, enter your name and email in the same section and select the letter type.
When finished configuring the Proposal, select ‘Save and Continue’ to proceed to the Employee Coverage Lines step.
